MEDICAL RECORDS
The change from paper based to electronic medical records provided doctors at the clinic with the ability to rapidly review the key medical problems of their patients at every visit. The Patient Summary page in each computerised record contains important information about current medications, current and past medical problems, immunisation status, family history, allergies and warnings of any particular factors that the treating doctor should know about such as pregnancy and breast feeding.
Medical record information needs to be kept current and patients are encouraged to report any changes in their situation such as medication changes, hospital admissions, operations or family history changes. Accurate patient records are a vital component of good current medical care and form the basis of Preventive Care as well.
Family history is of particular importance in Preventive Care where knowledge of a family history of heart disease or cancer alerts the doctor to the need for screening measures that can reduce the risk of these problems for patients.
